Rehabilitation Assistant

We are seeking a highly motivated and enthusiastic Rehabilitation Assistant to join our team at Lincolnshire Community Health Services NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient-centered care and enhancing the lives of local residents.

The successful candidate will be responsible for assisting patients to regain as much functional independence as possible through working on a patient rehabilitation goal approach. You will use your skills to carry out simple assessments, follow care plans, and prescribe routine equipment within the scope of the job and to a level of assessed competence.

• Assist patients to regain functional independence through a patient-centered approach
• Carry out simple assessments and follow care plans
• Prescribe routine equipment within the scope of the job
• Work collaboratively with the multidisciplinary team to deliver high-quality care

We are looking for a candidate with the following essential criteria:

• Health-related NVQ level 3 or equivalent
• 5 GCSEs grade C including Maths and English or equivalent
• Experience within a caring environment
• Experience of working within a rehab environment

Desirable criteria include experience of MDT working, good communication skills, and relevant clinical skills.
