About the Role
This exciting opportunity has arisen for a skilled and motivated Community/IC Nurse Team Practitioner to join our team at Torbay and South Devon NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients in their own homes.
Key Responsibilities
- To provide comprehensive nursing assessments and care planning for patients with complex health needs.
- To work autonomously as a practitioner, planning and implementing nursing treatment programmes that meet the individual needs of patients.
- To advise and educate patients and carers on health and wellbeing issues, promoting self-care and independence.
- To carry out specialist assessments, including medication reviews and prescribing, to ensure patients receive the best possible care.
- To work collaboratively with other healthcare professionals to coordinate patient care and promote effective communication.
- To monitor and evaluate care programmes, making adjustments as needed to ensure optimal patient outcomes.

Requirements
- Qualified Registered Nurse Level 1.
- BSc (Hons) in Community Health Nursing (District) or equivalent.
- Experience of working in a community nursing setting, preferably with a focus on intermediate care.
