Job Title: Care Activities Coordinator

We are seeking a highly motivated and experienced Care Activities Coordinator to join our team at Champion House. As a Care Activities Coordinator, you will play a vital role in creating a stimulating and varied programme of leisure activities and social events for our residents.

Key Responsibilities:

  • Lead and encourage residents to participate in meaningful activities, promoting their independence, choice, and dignity.
  • Create a programme of leisure activities and social events, both within and outside the home, to cater to the diverse interests and needs of our residents.
  • Work with new residents to understand their preferences, abilities, and needs, developing individual social care plans and regularly evaluating their effectiveness.
  • Engage with the local community to initiate contact with community groups, enabling residents to participate in or continue with their hobbies and interests.

About the Role:

This is a fantastic opportunity to join a dynamic team that specializes in supporting individuals with varying needs resulting from physical and long-term neurological conditions. Our teams are highly skilled and trained to deliver specialist and complex nursing care, with significant experience in tracheostomy, PEG care, wound management, and end-of-life care.

Requirements:

  • 6 months' experience as a Care Activities Coordinator or 1 year working as an experienced Support Worker/Healthcare Assistant/Community Carer.
  • Ability to work on a shift/rota pattern.
  • Right to work in the UK.
