Job Title: Palliative Care Specialist

We are seeking a highly skilled and compassionate Palliative Care Specialist to join our team at St Helena Hospice. As a key member of our Single Point team, you will provide high-quality care to patients and their families, working closely with community and primary care colleagues to ensure seamless care delivery.

Key Responsibilities:
  • Provide specialist palliative care to patients and their families, including assessment, planning, implementation, and evaluation of care needs.
  • Work as part of a multidisciplinary team to deliver high standards of care, including Hospice in the Home MDT and rapid response visits.
  • Triaging calls and providing advice and support to patients and professionals, including those known to St Helena or recorded on the My Care Choices Register.
  • Be a clinical resource to Clinical Support Workers, Health Care Assistants, and Administrators, providing guidance and support as needed.
  • Work closely with community and primary care colleagues, St Helena, and hospital staff to ensure patients receive an integrated service in a timely manner.
Requirements:
  • Full UK driving licence and willingness to improve knowledge and skills within palliative care.
  • Experience of working in the community and with people with mental health needs, sensory and learning disabilities.
  • Ability to accurately record information and maintain effective communication with patients, families, and healthcare professionals.
  • Flexibility to meet the needs of the service and work overnight, including triaging calls and visiting patients as necessary.
