Position Overview

Macmillan Palliative & End of Life Care Transition Specialist Nurse.

We are seeking a dedicated and proficient palliative/end of life care nurse to oversee the expedited discharge service.

This role is intended to provide maternity cover.

The ideal candidate will focus on delivering high-quality expedited discharges for all patients referred to Specialist Palliative Care services for discharge coordination. The primary responsibility will be to offer specialized assistance in effectively evaluating, planning, and facilitating the discharge of patients with limited life expectancy from the hospital environment into the community. This position is hospital-based and aims to support colleagues, patients, and their families. The expedited discharge service will enhance discharge coordination and enable more patients to pass away in their preferred location, striving to minimize the number of hospital deaths when feasible or suitable.

This role necessitates maintaining close, consistent, and ongoing connections with commissioners and other entities associated with the NHS funding process. It also involves collaborating closely with relatives and significant others to ensure a smooth and secure transition of care to the designated discharge location. The working hours for this position will be from 9 am to 5 pm, Monday to Friday.

Applicants with relevant experience may be considered for a secondment or part-time hours/flexible working; however, full-time fixed-term candidates will be prioritized.

Main Responsibilities

The Palliative and End of Life Care Transition Specialist will be expected to:

  • Conduct thorough assessments, plan, and deliver specialized care and support to facilitate the discharge of patients with limited life expectancy from the acute care setting to the community, collaborating as part of a multidisciplinary team and fostering positive working relationships with other healthcare professionals and service users.
  • Engage in holistic assessments in collaboration with other team members to ensure the delivery of high standards of specialized palliative care.
  • Adhere to statutory legislation and local policies and procedures related to discharge planning and palliative care.
  • Implement best practices in the discharge process by addressing the needs of this patient demographic, thereby optimizing the experience for both patients and caregivers.
  • Serve as a resource, educator, and advocate for patients, their caregivers, and healthcare professionals.
  • Operate within the framework of Trust policies and national guidelines.
  • Enhance patient and caregiver satisfaction at the end of life.
  • Contribute to improving the overall end-of-life experience across the Trust through education and support.
  • Facilitate prompt discharge processes for patients requiring palliative and end-of-life care.

Job Responsibilities

  • Collaborate within the team and across the hospital to provide ongoing support to optimize quality care for patients, their caregivers, and significant others with palliative and end-of-life care needs.
  • Offer specialized guidance and advice on complex palliative, end-of-life care, and patient discharge planning needs.
  • Act as the patients' advocate, assisting them in understanding their conditions and available treatment options, empowering them to make informed decisions regarding their care.
  • Ensure accurate and comprehensive documentation across all formats utilized within the Trust.
  • Proactively engage in the discharge planning of palliative and end-of-life patients, ensuring a seamless transition from hospital to community services.
  • Facilitate complex palliative/end-of-life care discharges for patients residing outside local boroughs by coordinating with relevant teams.
  • Demonstrate leadership through clinical expertise, delivering high standards of patient-centered care for those with palliative care needs.
  • Collect and analyze quantitative and qualitative data to provide evidence of productivity, outcomes, and quality, utilizing this data to support business cases and reports as necessary.
  • Collaborate with the lead palliative care team to identify service objectives and contribute to annual reports and work plans.
  • Engage with commissioners and agencies to secure funding for expedited discharges and work with all involved parties to ensure a safe discharge pathway for patients.
  • Promote best practices relevant to the needs of palliative/end-of-life care patients, utilizing current strategies and protocols to enhance quality of life.
  • Support appropriate outcome measures to demonstrate the quality of service provision.
  • Identify learning needs and develop, implement, and evaluate educational programs to address these needs.
  • Act as a facilitator/educator for relevant health and social care professionals regarding discharge needs in palliative and end-of-life care.
  • Participate in appraisals and regular one-on-one meetings with the line manager.
  • Maintain professional accountability and recognize personal limitations within the framework of professional practice.
  • Comply with and implement Trust policies and procedures.
  • Adhere to NMC and other regulatory requirements.

Qualifications and Skills

Essential

  • First level RGN registration.
  • Evidence of recent professional development.

Desirable

  • Educated to degree level or currently pursuing a degree.
  • Experience or willingness to engage in clinical supervision.

Skills and Abilities

Essential

  • Clinical leadership skills.
  • Demonstrated nursing experience post-registration in palliative or end-of-life care or in a related field.
  • Experience in teaching within the clinical environment.

Desirable

  • Strong presentation skills.
  • Experience in a key worker role.

Experience

  • Experience in teaching/educating nurses, allied health professionals, and/or patients.
  • Experience working in the healthcare sector with an interest in palliative care or related experience.
  • Understanding of the issues faced by palliative patients, families, and caregivers, and their role in service development.

  • Up-to-date knowledge of palliative care specialties and treatments.
  • Evidence of innovation and change management.
