Position Overview
An exciting opportunity has emerged for the role of Clinical Care Facilitator within the Recovery and Support Teams. We are looking to fill a nursing care coordination position that plays a vital role in our community mental health services.
The ideal candidate will have the opportunity to grow as a Clinician within a collaborative, multi-disciplinary environment. The Clinical Care Facilitator will report to the Team Manager, who in turn reports to the Clinical Manager for Adult Community Services.
Key Responsibilities
We seek a proactive, dedicated, and enthusiastic individual who embodies the Trust's values and contributes to a positive workplace culture. This position is open to those who are professionally registered with the relevant regulatory bodies. Experience in managing cases within a mental health community context is highly desirable.
The successful candidate will be accountable for managing a caseload and will engage in essential meetings aimed at delivering high-quality, recovery-oriented services to clients. The role also involves meeting established Key Performance Indicators (KPIs) related to service activity and quality.

Job Duties
- Operate independently within the scope of the role, guiding and mentoring junior staff and the broader care team to ensure the delivery of high-quality nursing care.
- Lead and educate junior staff on risk assessment and management, utilizing evidence-based practices and clinical judgment.
- Advocate for the needs and rights of service users, empowering them to take an active role in their care plans.
- Escort service users as necessary, adhering to organizational policies.
- Demonstrate a comprehensive understanding of mental health conditions and their manifestations, supporting junior staff in recognizing and responding to these effectively.
- Address challenging behaviors in accordance with organizational policies, leading the care team in managing difficult situations and fostering resilience.
