About the Role

We are seeking an experienced paediatric nurse to join our innovative Children's Home Acute Team (CHAT). As a key member of our team, you will provide excellent acute nursing care to acutely unwell children in their homes, working autonomously and as part of a dynamic community nursing team.

Main Responsibilities
  • Deliver high-quality acute nursing care in the community, working in partnership with families and other healthcare professionals.
  • Develop and implement individualized care plans to meet the unique needs of each child and their family.
  • Contribute to the development of existing ambulatory and unscheduled care pathways, preventing unnecessary admissions and facilitating early discharge from inpatient wards.
  • Work collaboratively with the CHAT team to provide comprehensive care to children and their families.
