About the Role

As a Community Care Supervisor at Sage Care, you will play a pivotal role in enhancing the quality of care services provided to our clients. Each day presents unique challenges and rewarding opportunities to make a significant impact on the lives of those we serve.

Your Responsibilities

In this role, you will:

  • Assist in delivering top-notch care support services through various field functions.
  • Conduct assessments and reviews of individual care services.
  • Provide practical supervision, guidance, and support to care workers.
  • Collaborate with Care Coordinators to develop effective care worker schedules tailored to individual needs.
  • Regularly review services, including needs and risk assessments.
  • Engage in multi-disciplinary reviews of service users' requirements as necessary.
  • Mentor new care workers in the field to ensure their proficiency.
  • Evaluate care worker performance through on-site observations and routine supervision.

While the majority of your time will be spent in the community, some office-based administrative tasks will also be part of your responsibilities.

Qualifications

What You Bring:

Experience in conducting initial risk and needs assessments for individual service users is essential. You should be adept at collaborating with service users, their families, caregivers, healthcare professionals, and advocates to ensure the seamless delivery of comprehensive care support.
