The Role- Support individuals with a wide range of needs and aspirations
- Work with our organisational values to encourage independence
- Support with personal care needs, daily activities and appointments within the local community
- Work closely with a small team to provide 24/7 support
- Develop and maintain positive relationships with service users and their families
- Assess and meet the individual needs of service users
- Support service users to access community resources and activities
- Collaborate with other professionals to ensure comprehensive support
