Job Title: Advanced Pharmacist for Community Services

We are seeking a highly skilled and experienced pharmacist to join our team as an Advanced Pharmacist for Community Services. This is a unique opportunity to work in a dynamic and supportive environment, providing clinical pharmacy services to West Cumberland Hospital and the surrounding community.

About the Role

The successful candidate will be responsible for providing high-quality clinical pharmacy services, including medication management, patient education, and clinical governance. You will work closely with our multidisciplinary team to ensure seamless care delivery and contribute to the development of our clinical pharmacy services.

Key Responsibilities
  • Provide clinical pharmacy services to patients in West Cumberland Hospital and the community
  • Develop and implement medication management plans to improve patient outcomes
  • Conduct patient education and counseling to promote medication adherence and safety
  • Contribute to clinical governance and quality improvement initiatives
  • Collaborate with the multidisciplinary team to ensure seamless care delivery
Requirements
  • MPharm or equivalent qualification
  • Registration with the General Pharmaceutical Council
  • Experience in acute or community hospital pharmacy
  • Independent prescribing qualification or commitment to achieving this
  • Highly developed communication and interpersonal skills
