Main Responsibilities
  • Develop and implement innovative services within general practice and primary care's interface with other parts of the health and social care system.
  • Deliver frontline patient care, leading by example on the delivery of prudent and value-based healthcare.
  • Manage patient caseloads, design and evaluate new professionally integrated services, and collaborate with multidisciplinary teams to integrate pharmaceutical care for optimising medication management and better patient experience and outcomes.
  • Lead on training and education initiatives for our current and future workforce, and develop or adopt best practices by contributing to research and quality improvement.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire, and its bordering counties. We provide primary, community, in-hospital, mental health, and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.

Person Specification
  • Essential:
    • Master's degree in Pharmacy
    • Registered pharmacist with GPhC
    • Post-graduate diploma/MSc in clinical pharmacy or equivalent experience
    • Independent Prescriber Qualification
    • RPS (Royal Pharmaceutical Society) credentialed Consultant Pharmacist (or working towards this)
  • Desirable:
    • Post-graduate studies in clinical specialty areas
    • Teaching / assessing qualification
Experience
  • Essential:
    • Significant evidence of autonomously clinically assessing and managing patients across a full spectrum of typical presentations in both acute and chronic disease
    • Significant evidence of educating, mentoring, and operating as a clinical supervisor across the healthcare system
    • Significant evidence of developing, conducting, and publishing pharmaceutical/pharmacy practice research
    • Evidence of policy and strategy development and change management in complex situations
    • Demonstrable evidence of implementing service re-design and delivering transformational change
    • Experience of working with Higher Education Institutes including course design and delivery of teaching
