About the Role

We are seeking a skilled and compassionate Adult Social Care Worker to join our Mental Health and Wellbeing Integrated Care Team (MINT) in North Locality.

Key Responsibilities
  • Conduct Care Act 2014 assessments and draw up care plans with individuals, identifying their strengths and person-centred outcomes.
  • Complete Mental Capacity Assessments under the Mental Capacity Act 2005 and be involved in Best Interest Decision making where appropriate.
  • Work alongside people to provide information and advice to enable them to use their own skills and assets to keep them well or to support their recovery.
  • Collaborate with other professionals, including nurses, psychologists, and the third sector, to engage with people and reduce support while maximising independence related to their mental health.

Requirements

This role is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check. A valid driving licence and own transport, or a suitable alternative method of travel, is an essential condition for this role.
