Job Summary

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our team at Lincolnshire Community Health Service NHS Trust. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and working autonomously to assess, diagnose, and treat patients with complex undifferentiated or undefined presentations.

Main Responsibilities
  • Assessment and Diagnosis: Use your highly developed clinical knowledge and skills to assess, diagnose, and treat patients with complex undifferentiated or undefined presentations.
  • Physical Examinations and Investigations: Perform physical examinations, analyse complex clinical signs and investigation results, and diagnose conditions, initiating treatment as required.
  • Requesting Investigations: Request investigations, such as blood, urine, and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, and X-Rays, in accordance with IR(ME)R regulations and local policy.
  • Formulating Management Plans: Formulate management plans, make clinical decisions, and develop complex treatment plans to manage acute illness and acute episodes of chronic illness, including referral, admission, or discharge.
  • Providing Clinical Cover: Provide clinical cover to departments within the organisation, according to patient and staffing needs.
  • Prescribing and Reviewing Medication: Prescribe and review medication, as an independent prescriber, for therapeutic effectiveness, appropriate to patient need, and in accordance with best/evidence-based practice and national and local protocols.
Key Skills and Qualifications
  • Advanced Clinical Practice Qualifications: Recognised Advanced Clinical Practice Qualifications at Masters Level.
  • Urgent Care Experience: Proven and current experience in urgent care/emergency medicine settings.
  • Professional Development: Evidence of recent professional development in the past year and demonstration of seeking and undertaking autonomous learning and development opportunities.
  • Training and Education: Ability to identify training needs in other healthcare professionals and formulate detailed teaching plans.
  • Critical Analysis and Application: Ability to critically analyse published research and apply theory to practice.
