We are seeking a highly skilled and experienced Dementia Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing specialist dementia care and support to individuals and their families in the community.
Key Responsibilities- Provide expert dementia care and support to individuals and their families in the community.
- Work closely with GPs, care homes, and other healthcare professionals to ensure seamless care coordination.
- Develop and implement personalized care plans to meet the unique needs of each individual.
- Provide education and training to healthcare professionals and the wider community on dementia awareness and care.
- Contribute to the development and implementation of dementia care policies and procedures.
- Professional mental health qualification and registration (e.g., RMN, Social Work Degree, or equivalent).
- Substantial experience of managing clinical complexity in a community setting.
- Excellent communication and interpersonal skills, with the ability to engage with people experiencing a wide range of needs related to dementia.
- Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
- Computer literacy and ability to use IT systems, including electronic record keeping.
