About the Role

We are seeking a highly skilled and dedicated Learning Disability Inpatient Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with learning disabilities and complex mental health needs.

Key Responsibilities
  • Assess and develop individual care plans to meet the unique needs of each patient
  • Provide leadership and management to the inpatient unit, ensuring safe and effective care delivery
  • Collaborate with multidisciplinary teams to develop and implement Positive Behavioural Support Plans
  • Conduct risk assessments and develop strategies to mitigate risks
  • Ensure accurate and timely recording of patient information and progress
Requirements
  • Registered Nurse RNLD/RNM or dual qualified Registered Nurse RNLD and Social worker integrated practice
  • Degree/HE Diploma level
  • Experience in primary/ named nursing, shift management, and assessment and care planning for individuals with learning disabilities
