Job Overview

We are seeking a highly skilled and experienced Learning Disability Nurse to join our team as a Community Specialist Practitioner. The successful candidate will be responsible for supporting the delivery of the new Young People's Specialist Support Team service to children and young people across the Pennine Care footprint.

This is a challenging and rewarding role that requires a strong understanding of Positive Behavioural Support Principles and the ability to work effectively with children and young people with complex needs, including autism and behaviour that challenges.

Main Duties of the Job
  • Assess and formulate individualised care plans for children and young people with complex needs
  • Develop and implement Positive Behavioural Support plans to promote positive behaviour and reduce challenging behaviour
  • Work collaboratively with other professionals, including clinical psychologists, occupational therapists, and support workers, to provide a comprehensive service to children and young people
  • Manage risk and develop risk management plans to ensure the safety and well-being of children and young people
  • Provide training and support to other professionals to promote best practice in Positive Behavioural Support

The successful candidate will have a strong understanding of Positive Behavioural Support Principles and the ability to work effectively with children and young people with complex needs.

Person Specification
  • Relevant health or social care qualification with current professional registration
  • Experience of working with children and young people with complex needs, including autism and behaviour that challenges
  • Strong understanding of Positive Behavioural Support Principles and the ability to develop and implement Positive Behavioural Support plans
  • Ability to work collaboratively with other professionals and manage risk
  • Excellent communication and interpersonal skills
