Lancashire County Council is seeking a dedicated and compassionate Adults Social Worker to provide essential support to adults in the community. This role offers the opportunity to make a real difference in the lives of adults, providing support during key transitions in their lives.

Key Responsibilities:
  • Hospital Discharge: Coordinate and facilitate safe and timely discharges from hospitals, ensuring all necessary support is in place for a smooth transition back to the community or to appropriate care settings.
  • Triage/Duty Services: Respond to urgent cases, assess needs, and make informed decisions to provide the right level of support at the right time.
  • D2A Reviews: Conduct Discharge to Assess reviews, assessing individuals in their own homes or care settings to determine ongoing care and support needs.
  • CHC MDTs: Participate in Continuing Healthcare Multi-Disciplinary Team meetings to assess and review eligibility for CHC funding and to ensure appropriate care plans are in place.
  • Hospital Avoidance: Work proactively to prevent unnecessary hospital admissions by providing timely interventions and support in the community.
Requirements:
  • A degree in Social Work and registration with Social Work England.
  • Experience in adult social care, particularly in hospital discharge, triage/duty, or similar roles.
  • Access to your own car.
  • Enhanced DBS.
