Key Responsibilities
The Healthcare Practice Nurse will be accountable for delivering nursing services within the practice, collaborating with a multidisciplinary team to provide care tailored to our patient population. Under the guidance of senior nursing staff, the role focuses on:
- Facilitating health and wellness initiatives while preventing health-related issues.
- Monitoring chronic conditions and engaging in health promotion and screening efforts.
- Working cohesively with the general practice team to fulfill patient needs and uphold established policies and procedures.
- Providing leadership in nursing practices as necessary.
About Ammonite Health Partnership
Ammonite Health Partnership consists of three practices, each contributing to a collaborative healthcare environment. Our team includes a diverse group of healthcare professionals, ensuring comprehensive care for our patients.
Core Duties
The primary responsibilities of the Healthcare Practice Nurse include, but are not limited to:
- Assessing and implementing health promotion strategies.
- Developing and managing treatment plans for patients with chronic conditions.
- Prioritizing health concerns and providing timely interventions.
- Encouraging self-care and healthy lifestyle choices among patients.
- Administering medications and managing prescriptions in accordance with regulations.
- Conducting health assessments and supporting women's health initiatives.
