About the Role

Morrab Surgery is seeking an experienced and skilled Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in providing high-quality nursing care to our patients, working closely with our General Practitioners and other healthcare professionals to deliver evidence-based practice.

Key Responsibilities

  • Provide and maintain a high standard of nursing care to patients with long-term conditions and acute problems, incorporating evidence-based practice.
  • Support the Infection Prevention Control (IPC) lead in implementing audits, training, and evidence of compliance.
  • Develop and implement an effective care management programme for frail and housebound patients.
  • Assess patient needs and provide healthcare that is appropriate, incorporating evidence-based practice.
  • Develop and implement health promotion and wellbeing programmes.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients in accordance with clinical-based evidence, NICE, and NSF.
  • Diagnose and manage acute and chronic conditions as required.
  • Review patient medications to enhance compliance.
  • Provide guidance to patients on the use of prescribed and over-the-counter medications.
  • Coordinate the provision of travel medicine services.
  • Request pathology services and process pathology results as required.
  • Oversee the provision of chronic disease clinics and deliver patient care as necessary.
  • Maintain accurate clinical records in conjunction with existing legislation.
  • Ensure the appropriate use of chronic disease registers.
  • Develop, implement, embed, and participate in well-woman and well-man clinics.
  • Ensure compliance with the practice chaperone policy.
  • Assist GPs with minor surgery when required.
  • Prioritize health issues and intervene appropriately.
  • Support the team in dealing with clinical emergencies.
  • Recognize, assess, and refer patients presenting with mental health needs.
  • Implement vaccination programmes for adults and children.
  • Liaise with external services/agencies to ensure patient support.
  • Delegate clinical responsibilities appropriately, ensuring safe practice.
  • Support the clinical team with safeguarding matters.
  • Understand practice and local policies for substance abuse and addictive behaviour.
