Key Responsibilities- Manage a caseload of individuals, creating and implementing personalized recovery plans to support their progress.
- Facilitate and promote the progression of individuals from initial engagement into structured treatment programs.
- Contribute to reducing substance-related harm to individuals and the wider community through evidence-based interventions.
- Collaborate closely with key partners and colleagues to achieve the best health and wellbeing outcomes for individuals.
- Apply co-production principles to support individuals in making informed decisions about their treatment and recovery.
- Think creatively about treatment options and transitions, working collaboratively with the multi-disciplinary team to provide intensive and co-created recovery solutions.
