About the Role

We are seeking a highly skilled and motivated Physiotherapist to join our Adults Community team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality Physiotherapy support to our patients.

Key Responsibilities
  • Develop and deliver Physiotherapy services to prevent and manage long-term conditions, including obesity, hypertension, and diabetes.
  • Work collaboratively with our interdisciplinary teams to meet the needs of patients, supporting the delivery of our Frailty framework and LTC workstream.
  • Provide clinical education and leadership, upskilling our local workforce and promoting best practice.
  • Contribute to the development and delivery of our frailty framework, ensuring high-quality care for our patients.

Requirements
