Key Responsibilities
- Collaborate with Advanced Practitioners within the neighbourhood proactive care framework, assisting complex patients to facilitate their care at home.
- Work closely with the Advanced Practitioner as part of a cohesive team.
- Enhance your skills in first contact care, providing thorough assessments for patients with undiagnosed conditions or worsening existing health issues.
- Function as an independent practitioner under the guidance of the Advanced Practitioner, delivering clinical assessments and treatments to a varied caseload of adults requiring urgent or ongoing care.
- Utilize your expertise to ensure safe, high-quality, evidence-based care for patients receiving urgent care or those on a neighbourhood caseload.
- Engage collaboratively with all health and social care teams within the community, referring patients based on their clinical or social requirements.
- Work in partnership with acute and primary care colleagues, accepting referrals for patients who may benefit from the service.

Job Responsibilities
- Assume direct responsibility for patient care, including monitoring and diagnostic assessments.
- Act as an independent practitioner under the leadership of the Advanced Nurse Practitioner, providing clinical evaluations and treatments within your professional scope.
- Conduct initial assessments, request and interpret diagnostic tests, and propose differential diagnoses with guidance from the clinical lead or Advanced Practitioner.
- Triage and prioritize patients, employing various techniques to gather medical histories and formulate appropriate care plans.
- Coordinate intensive care packages and offer clinical guidance on care management.
- Identify changes in patient conditions through clinical evaluations and take necessary actions to ensure patient safety.
- Implement evidence-based interventions, applying knowledge and skills to meet patient needs.
- Make referrals for diagnostic tests and initiate investigations to support clinical decisions.
- Evaluate the effectiveness of interventions against previously established goals and adjust plans as necessary.
- Conduct assessments and, as a non-medical prescriber, prescribe within your professional scope.
- Ensure that care and services align with Stockport NHS Foundation Trust policies and the Clinical Governance Framework.
- Maintain accurate and timely patient records in accordance with Trust and national guidelines.
- Utilize clinical skills and expertise to provide proactive monitoring and timely interventions.
- Recognize vulnerabilities within the patient and carer caseload and report potential safeguarding concerns as per local policies.
- Collaborate with multidisciplinary teams to develop, implement, and evaluate care plans for patients and their caregivers, ensuring their safety while enabling them to remain at home.
- Take responsibility for creating Enhanced Case Management Plans to minimize unnecessary hospital admissions.
- Educate and empower patients and caregivers to recognize health deterioration and manage their conditions effectively.
The Trust is dedicated to safeguarding and promoting the welfare of children, young people, and vulnerable adults, expecting all staff and volunteers to uphold this commitment by adhering to Trust policies and procedures.
Successful candidates will be required to undergo an enhanced DBS check, with associated costs deducted from the first salary.
Additionally, joining the DBS update service and paying the annual subscription fee is a condition of employment.
Qualifications
Essential
- Registered Nurse (Adult) with NMC.
- Degree or equivalent experience.
- Completion of Advanced Clinical Assessment module at Level 7 or willingness to undertake.
- Non-Medical Prescribing (NMP V300) or willingness to undertake.
Knowledge
Essential
- Current professional portfolio demonstrating competencies and reflective practice.
- Understanding of long-term condition management and elderly care.
- Awareness of broader health and social care systems beyond immediate role.
- Familiarity with the expectations of the Prevention of Admission service, particularly regarding patient outcomes.
Experience
Essential
- Post-registration experience at a senior level in the specialty.
- Experience in community nursing.
Desirable
- Experience in comprehensive geriatric assessments.
