Position Overview

Role: Community Support Facilitator
Service: Housing & Community Support Services

Base: The Action Group, Regional Office

Reporting to: Practice & Wellbeing Leader

Compensation: Casual rate, £12 per hour; Sleepover rate (if applicable) £96 per night.

Work Schedule: Casual

Objective:

The primary objective of this role is to deliver a compassionate, efficient, and effective 24/7 support service to individuals utilizing The Action Group's offerings. This includes aiding individuals in skill development by supervising and engaging in various support activities that enhance their opportunities and promote autonomy.

Key Responsibilities

Specific Responsibilities
  • Implement the TAG Key Worker framework, including organizing support plans and case reviews in collaboration with relevant stakeholders, ensuring smooth transitions between staff, and maintaining accurate records.
  • Foster the physical and emotional well-being of individuals at all times (e.g., assisting individuals in maintaining appropriate self-care levels, conducting risk assessments).
  • Build meaningful and trusting relationships with individuals (e.g., assisting individuals in understanding their behaviors and reactions).
  • Encourage independence in daily living activities, providing support only when necessary (e.g., household chores, meal preparation, administrative tasks).
  • Promote individuals' hobbies, interests, and community involvement, including organizing outings and accompanying individuals to various activities.
  • Provide personal care assistance as required.
  • Support individuals in enhancing their communication skills and advocate for self-advocacy whenever possible.
  • Act as an advocate for individuals when necessary (e.g., liaising with external organizations regarding benefits, education, and employment).
  • Collaborate with family members and caregivers of individuals (e.g., organizing support plans, addressing concerns) to ensure their involvement in the individual's life.
  • Research local services and amenities available to individuals.
  • Engage with both individuals and groups as needed.
  • Participate flexibly in various shift patterns required by the needs of individuals, including evening and weekend work as applicable.
  • Occasionally undertake 'sleepover' or 'waking night' duties as needed.
  • May be asked to cover short-term absences of Support Workers in other teams.

General Responsibilities

  • Understand and work in alignment with TAG's Mission, Aims & Objectives, and current Business Plan.
  • Effectively manage personal work time.
  • Maintain organized filing systems and keep up with necessary paperwork.
  • Treat all individuals with respect and consideration.
  • Ensure clear and prompt communication, whether addressing inquiries from colleagues or concerns from individuals.
  • Stay informed and comply with all relevant TAG policies and procedures.
  • Uphold individual rights and responsibilities at all times.
  • Maintain confidentiality as appropriate to the role.
  • Assist managers and colleagues as needed.
  • Take responsibility for personal Continuous Professional Development (CPD) and contribute to maintaining TAG's Investors In People standard.
  • Manage work-life balance effectively.
