The Role: Support Worker - Outreach & Supported Living

We are seeking compassionate and dedicated Support Workers to join our team in Sheffield. In this role, you will play a vital part in enhancing the lives of adults with learning disabilities, autism, mental health conditions, and physical challenges.

Your Responsibilities:

  • Provide personalized support to individuals, assisting them in achieving their goals and living as independently as possible.
  • Facilitate meaningful activities and life skills development, including personal care, budgeting, shopping, medication management, cooking, cleaning, and attending appointments.
  • Encourage social engagement and community participation, fostering a sense of belonging and purpose.
  • Collaborate with families, healthcare professionals, and other stakeholders to ensure comprehensive and coordinated care.
