Job SummaryWe are looking for a Support Practitioner to provide personalised support to individuals in their own homes, creating an environment that promotes their well-being and independence. As a Support Practitioner, you will work collaboratively with our team to ensure the safety, well-being, and fulfillment of the individuals we support.
Key Responsibilities- Provide personalised support, including personal care, housework, medication administration, meal preparation, financial management, and companionship.
