Job Summary

We are looking for a Support Practitioner to provide personalised support to individuals in their own homes, creating an environment that promotes their well-being and independence. As a Support Practitioner, you will work collaboratively with our team to ensure the safety, well-being, and fulfillment of the individuals we support.

Key Responsibilities
  • Provide personalised support, including personal care, housework, medication administration, meal preparation, financial management, and companionship.
  • Assist individuals in their day-to-day lives while empowering them to pursue happiness and fulfillment, both within their homes and in the broader community.
  • Develop and nurture trusting relationships, which serve as the foundation for effective support.
  • Collaborate with families and professionals to deliver top-quality care tailored to each individual's needs.
  • Advocate for independence and honor the uniqueness of each personal Support Plan.
  • Maintain up-to-date records of daily tasks and support provided.
