Sanctuary Personnel Ltd is seeking a highly skilled and experienced Advanced Healthcare Practitioner to join our Safeguarding Team on a permanent basis. The successful candidate will be based in Northamptonshire and will work full-time.
Key Responsibilities- Support and Guidance: Provide support and guidance to senior colleagues in the prioritization, allocation, planning, and review of work, ensuring effective budget management and maximizing service efficiencies.
- Service Monitoring and Evaluation: Contribute to service monitoring and evaluation, undertaking needs analysis, reviewing availability of resources, and developing service practice, policies, and strategies to maintain high-quality services.
- Supervision and Direction: Provide supervision, direction, and coordination of workloads, support, and guidance, appraisal, and professional development, and support social work students on first and final placements, and students from other professional disciplines.
- Service User Assessments and Interventions: Undertake clear and detailed service user, carer assessments, and interventions in accordance with relevant statutory requirements, minimizing risks, with consideration for service user needs and available options.
- Case Management: Manage high-complex cases, including planning and review, promoting independence and early intervention, as well as the personalization agenda and self-directed support to minimize risks and improve quality of life for service users and their carers, ensuring targets are met within agreed timescales.
- Community Engagement: Work with individuals, families, carers, and communities to help them make informed decisions, enabling them to clarify their needs, construct value for money care packages, and commission a range of appropriate services, as well as respond to their feedback regarding service provision.
- Statutory Responsibilities: Assist in ensuring that statutory responsibilities are met and fulfilled by the team, ensuring that quality practice standards are achieved in delivering high-quality services, promoting the use of evidence and theory to support practice in complex and changing circumstances.
- Preventative Work and Risk Assessments: Undertake preventative work and risk assessments, utilizing community resources and services such as reablement and telehealthcare.
- Complex Transitions: Manage complex transitions cases and chair multi-disciplinary team meetings as appropriate to specialized field/local structures.
- Professional Development: Maintain up-to-date knowledge of best practice, policy, and procedure in relation to vulnerable people, as well as keeping abreast of wider service developments to enhance the service accordingly to satisfy internal and external demands.
- Travel: Ability to travel independently to all required areas by the job's responsibilities.
