Job Title: Community Nurse

We are seeking a compassionate and dedicated Community Nurse to join our team at Unity Healthcare Recruitment Ltd.

About the Role:

As a Community Nurse, you will play a vital role in delivering high-quality care and support to patients in their homes. Your primary responsibility will be to provide exceptional nursing care, promoting independence and well-being among our patients.

Key Responsibilities:
  • Conduct thorough assessments of patients' needs and develop personalized care plans.
  • Deliver high-quality nursing care, including medication management, wound care, and vital sign monitoring.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Valid Nursing and Midwifery Council (NMC) registration.
  • Proven experience as a Community Nurse or similar role.
  • Ability to work autonomously and within a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Must be a car driver with a valid driving license.
