Job Title: Specialist Clinical Pharmacist

About the Role

We are looking for a Specialist Clinical Pharmacist to work as part of our clinical pharmacy team, developing services across the patient pathway to ensure continuity of care and promote better health outcomes.

Key Responsibilities
  • Carry out polypharmacy reviews in collaboration with medical colleagues to positively contribute to patient care.
  • Work at a senior level within the service, playing an influential role in the patient's care pathway.
  • Develop and implement clinical pharmacy services to improve patient outcomes.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery.
Requirements
  • Previous experience working in a clinical environment.
  • Post-graduate diploma in clinical pharmacy.
  • Registration with the General Pharmaceutical Council.
  • Prescribing qualification or willingness to work towards one.
