Job Title: Community Health Nurse

We are seeking a highly skilled and dedicated Community Health Nurse to join our team at Transformation Unit. As a Community Health Nurse, you will play a vital role in promoting the health and well-being of children and families in our community.

Key Responsibilities:
  • Conduct health needs assessments and screenings for children, providing prompt feedback and escalating concerns as necessary.
  • Participate in the planning, delivery, and evaluation of health promotion activities, utilizing evidence-based practice to support individuals, families, groups, and communities.
  • Contribute to the Early Help offer, supporting families to access additional support as required in line with the Early Help framework.
  • Contribute to the safeguarding of children at risk, adhering to appropriate Safeguarding policies and procedures.
  • Participate in and promote childhood immunization programs as appropriate.
  • Engage in the Trust's staff nurse development program and other related Trust-based learning opportunities.
  • Work within the NMC code of professional conduct.
  • Support Health Visiting Teams in the management of children with additional medical needs, completion of Educational & Health care plans, and preparing/supporting families for transition to education.
  • Support and deliver evidence-based packages of care for children in line with the BFS Standard Operational Procedures for Health Visiting and clinical pathways.
  • Liaise with other Multi-Disciplinary Teams to ensure effective communication across the teams.
  • To support the Health Visiting and Early Years teams to deliver Early Help intervention and complete Early Help Plans as required.
  • To maintain accurate and contemporaneous electronic patient records for children and families, adhering to NMC requirements and the Trust's Record Keeping policies and guidelines.
  • To convey information of a complex nature to other professionals and clients with well-developed oral communication skills.
  • Undertake clinical audits as directed by Health Visitor/Health Visitor Team Leader, supporting action plans according to findings.
  • Interpret difficult/challenging situations, take prompt action, and have the ability to diffuse potentially aggressive or threatening situations.
  • Work with and support families through difficult family situations such as illness and bereavement.
  • To plan, organise and run clinics to support the health and developmental needs of children as required.
  • Ensure data is inputted onto a data collection system accurately and on time as required by the Trust.
  • To deliver care in various settings, including children's centres, clinics, GP surgeries, and clients' homes.
  • To ensure effective working relationships and communication with the BFS team and other professionals.
  • To maintain and continually update professional knowledge and skills, attending mandatory training and relevant updates training sessions as required by the Trust.
  • To co-operate and assist in research and survey programmes relating to the client group.
  • To participate in the teaching/training and assessment of support staff within the cluster.
Requirements:
  • First Level Registered Nurse
  • Evidence of relevant professional development post nurse registration
  • RSCN/Child Branch or Learning Disabilities
