About the Role

We are seeking an experienced Registered General Nurse (RGN) to join our team as a Clinical Nurse Specialist. As a key member of our homecare team, you will be responsible for delivering high-quality patient care and support to individuals in their own homes, workplaces, or communities.

Key Responsibilities:

  • Deliver clinical care and support to patients with complex medical conditions, including home parenteral nutrition, chemotherapy, IV antibiotics, and enzyme replacement therapy.
  • Work collaboratively with patients, families, and healthcare professionals to develop and implement personalized care plans.
  • Provide education and training to patients and their caregivers on medication management, wound care, and other health-related topics.
  • Conduct regular assessments and evaluations to ensure patients are receiving the highest level of care and support.
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and communication.
