We are seeking a dedicated and experienced Qualified Social Worker to join our Adults Learning Disability team in the London Borough of Sutton.
This is a fantastic opportunity to make a real impact in adult services and work closely with Children's Services, Education, and Health to provide essential support for young people with complex needs.
Key Responsibilities:- Conduct comprehensive assessments under the Care Act 2014 to ensure young people receive the appropriate support.
- Develop person-centred support plans that meet the changing needs of individuals and their families.
- Collaborate with NHS partners to secure Continuing Healthcare (CHC) funding for eligible individuals.
- Monitor and review support plans to ensure they meet the required standards.
- Safeguard the welfare of young people by adhering to statutory frameworks and legislation.
- Manage a varied caseload, making sound professional judgments and working closely with multidisciplinary teams.
- Supportive Environment: Regular professional supervision, comprehensive induction, and training opportunities.
