We are seeking a highly skilled and experienced Team Leader to join our team at Southernbrooks Community Partnerships, a leading organization in the NHS sector.

Job Summary

The successful candidate will be responsible for leading a small team of Social Prescribing Link Workers within a Primary Care Network (PCN). This is a challenging and rewarding role that requires strong leadership, coaching, and management skills.

Main Responsibilities
  • Leadership and Management: Provide line management and supervision to a small team of Social Prescribing Link Workers within Network 4 PCN.
  • Service Development: Assist Link Workers in building relationships with GP surgeries to establish a social prescribing service within the surgery.
  • Performance Management: Review the performance of individual Link Workers and the team against key performance indicators.
  • Risk Management: Ensure Link Workers risk assess cases and follow the appropriate safeguarding policies and procedures.
  • Data Collection: Collect and collate data to enable effective review of the service.
Requirements
  • Leadership Experience: At least 2 years experience of working with people with health and social needs, or community-based work with socially excluded groups.
  • Education: Educated to GCSE Grade C or above (or equivalent) in English and Maths.
  • Qualifications: NVQ level 2 in social care or equivalent experience.
  • Skills: Excellent organisational and time management skills, good IT skills, and experience of using databases to record client actions and interventions.
