The successful candidate will possess the necessary qualifications and expertise, working within established patient care protocols where applicable.
Key Responsibilities
- Conduct comprehensive health assessments for patients with chronic conditions, developing personalized management plans during annual evaluations, in collaboration with patients and other healthcare professionals, adhering to current guidelines and standards.
- Perform thorough medication evaluations and assessments for chronic disease management.
- Provide education, guidance, and support to both newly diagnosed and ongoing chronic disease patients.
- Conduct NHS Health Checks and New Patient Assessments.
- Assist in clinics for IUD fittings.
- Analyze urine samples for diagnostic purposes.
- Report on blood test results (training and protocols will be provided).
- Assess and administer travel vaccinations.
- Administer B12 injections.
- Provide Depo injections.
- Conduct ECG tests.
- Perform cervical smear tests.
- Carry out pill checks and HRT assessments.
- Order and administer necessary immunizations and vaccinations.
- Liaise with Child Health and Department of Health regarding immunization inquiries and programs.
- Ensure the maintenance of cold chain and monitor fridge temperatures.
- Collaborate on equipment management.
About Heaton Medical Centre
We are a training practice comprising a team of 7 GP partners, 2 salaried GPs, 2 Advanced Nurse Practitioners, GP trainees, paramedics, pharmacists, and practice nurses, serving a patient population of approximately 11,000.
Job Duties
In alignment with the NMC Code of Conduct, the post-holder will deliver assessment, treatment, screening, and health education services to patients. Accurate record-keeping in accordance with NMC standards is essential. The role contributes to the practice's compliance with GMC contract obligations.
The successful candidate will adhere to practice-specific protocols and guidelines, providing nursing care to patients both independently and in collaboration with nursing colleagues and general practitioners.
The post-holder will possess the required qualifications and expertise, working within established patient care protocols where applicable.
Qualifications
Essential
- Current Nursing Registration with the NMC.
- Educated to degree level or equivalent.
Desirable
- Non-medical Independent Prescriber qualification.
- Phlebotomy certification.
