About the Role

We are seeking a highly skilled and compassionate Care Team Leader to join our team at Ayton House in West Sussex. As a Care Team Leader, you will play a vital role in ensuring the highest standard of care for our residents.

Key Responsibilities
  • Lead and supervise your shift, ensuring that all care plans are implemented and residents receive the best possible care.
  • Case manage residents and liaise with families, healthcare professionals, and other stakeholders to ensure that care plans are effective and delivered.
  • Administer and record medication, ensuring that all medication is given safely and accurately.
  • Carry out risk assessments, identifying potential risks and taking steps to mitigate them.
  • Work alongside care assistants to support residents with daily needs, promoting independence and activity.
  • Work closely with the leadership team to develop services, ensuring that we are providing the best possible care for our residents.
  • Provide a fun, inclusive, and professional approach at all times, promoting a positive and supportive environment for residents and colleagues alike.
Requirements
  • NVQ Level 3 in health and social care or equivalent experience.
  • Proven ability to lead and supervise others, ensuring that all team members are supported and empowered to deliver high-quality care.
  • Good understanding of care planning processes and ability to implement care plans effectively.
  • Experience in administering medication or willingness to undertake training.
  • Ability to work well in a team, promoting a positive and supportive environment.
  • Strong communication and interpersonal skills, able to build relationships with residents, families, and colleagues.
