We are seeking a highly skilled and experienced Senior Community Nurse to join our team at Your Healthcare CIC. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients in their own homes and within the practice environment.
Key Responsibilities- Assess, plan, implement, and evaluate programs of care for service users and their carers.
- Work collaboratively with the wider multi-disciplinary team to provide the best care and quality outcomes for patients and service users.
- Manage a caseload of patients, attached to one or two GP surgeries, including case management of patients identified as being at risk of hospital admissions.
- Act as a role model, providing high-quality care and skilled assessment of clients' needs.
- Support team members in developing their skills and providing feedback through 1-1 meetings and appraisals.
- Registered General Nurse with relevant post-registration study at diploma or degree level.
- ENB 998 or equivalent Mentor or Practice Assessor course.
- Minimum 2 years community experience with demonstrable clinical expertise at Band 5 community nursing level.
- Experience of leading a team to achieve results, working within a multi-disciplinary team, and managing change within a complex environment.
- Excellent communication, interpersonal, negotiation, and presentation skills, including report writing.
