Key Responsibilities- Deliver personalized care to patients in their own homes, ensuring their dignity, comfort, and well-being are maintained at all times.
- Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
- Provide emotional support and practical assistance to patients and their families, promoting independence and confidence.
- Conduct regular assessments and reviews to ensure the quality of care is maintained and updated as needed.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing training and professional development to enhance skills and knowledge in end-of-life care.
- Care Certificate or NVQ Level 2 qualification in Health and Social Care.
- 3 years of experience in a similar role, with a proven track record of delivering high-quality care in a community setting.
- Current driving license and use of own vehicle.
- Knowledge of and experience in care at the end of life and after death.
- Current knowledge of Moving and Handling, Health and Safety, Safeguarding, and Mental Capacity Legislation and Guidance.
