About the Role

We are seeking a dedicated and compassionate Support Worker to join our Health and Local Crisis Response service. As a key member of our team, you will provide practical and emotional support to individuals in need, enabling them to live healthier and more fulfilling lives.

Key Responsibilities:

  • Provide reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Respond to referrals from Clinicians and Community Health Professionals.
  • Assess service users' needs, complete and follow a support plan for the individual.
  • Support service users in the weeks after hospital discharge or to prevent readmission.
  • Provide transportation to hospital appointments and assist with daily tasks such as shopping and prescription collection.

Requirements:

  • Full UK manual driving licence.
  • Good knowledge of services provided by the NHS and Social Care.
  • Ability to work flexibly, including evenings and weekends.
  • Professional and diplomatic communication skills.
