The Geriatric Care Team is in search of a dedicated, motivated, and enthusiastic individual to join our team at the Weston Division, part of University Hospitals Bristol and Weston NHS Foundation Trust, in the capacity of an Advanced Clinical Pharmacist – Band 8a.
The Geriatric Care Team comprises a diverse group of professionals collaborating across Weston General Hospital, primarily within the Geriatric Units of the Medicine Division. You will work alongside various partners, including community teams, ambulance services, and the voluntary sector, to ensure optimal care for our patients.

Key Responsibilities:
- Provide expert guidance, information, and recommendations to clinicians in frailty and emergency medicine regarding medication usage, expenditure, risk management, and compliance with medicines legislation.
- Collaborate with members of the Geriatric Care teams to develop protocols and guidelines related to medication use in the emergency department.
- Engage with external agencies, community pharmacies, and care home staff to ensure the safe, effective, and economical provision of medications for frail patients.
- Assess medications for patients referred to the Geriatric Care team and formulate a strategy for de-prescribing unnecessary medications.
- Coordinate with care home pharmacists to enhance pharmaceutical care for Geriatric patients across the primary-secondary care interface.
- Conduct research and development initiatives aimed at optimizing medication management for frail patients.
