About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our community team in Middlesbrough. As a Healthcare Support Worker, you will play a vital role in providing high-quality care and support to patients with life-limiting illnesses and their families.

Key Responsibilities
  • Deliver person-centered care to patients in their own homes, focusing on promoting independence and quality of life.
  • Assist patients with daily living activities, such as bathing, dressing, and medication management.
  • Monitor patients' symptoms and provide emotional support to patients and their families.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Provide education and support to patients and their families on end-of-life care and palliative care.
Requirements
  • National Vocational Qualification (NVQ) Level 2 in Health and Social Care or equivalent.
  • 6 months of care delivery experience in a health or social care environment.
  • Basic Food Hygiene Certificate (desirable).
  • Recent professional experience of providing care and caring for dying patients (desirable).
