Job Title: Rehabilitation Assistant

We are seeking a highly motivated and enthusiastic Rehabilitation Assistant to join our team at Lincolnshire Community Health Services NHS Trust. As a Rehabilitation Assistant, you will play a vital role in supporting patients to regain their independence and achieve their rehabilitation goals.

About the Role

The successful candidate will work closely with our multidisciplinary team to provide high-quality patient care and support. You will be responsible for assisting patients with their rehabilitation goals, including simple assessments, following care plans, and prescribing routine equipment. You will also work independently at times, under the professional guidance of occupational therapists and physiotherapists.

Key Responsibilities
  • Assist patients to regain their independence and achieve their rehabilitation goals
  • Work closely with the multidisciplinary team to provide high-quality patient care and support
  • Conduct simple assessments and follow care plans
  • Prescribe routine equipment within the scope of the job and to a level of assessed competence
  • Work independently at times, under the professional guidance of occupational therapists and physiotherapists
Person Specification

We are looking for a candidate with the following essential criteria:

  • Health-related NVQ level 3 or equivalent
  • 5 GCSEs grade C including Maths and English or equivalent
  • Experience within a caring environment
  • Experience of working within a rehab environment

Desirable criteria include:

  • Experience of MDT working
  • Good communication skills, written and verbal
  • Relevant clinical skills
