About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Lincoln. As a Support Worker, you will play a vital role in delivering outreach support to our customers with mental health conditions, enabling them to live the lives they choose.

You will work closely with our customers to understand their needs and preferences, providing personalized support to help them achieve their goals. This may involve assisting with daily living tasks, such as personal care and hygiene, medication administration, and domestic duties.

As a Support Worker, you will also be responsible for maintaining accurate records and updating care plans as required. You will work collaboratively with our team to ensure that our customers receive the best possible support and care.

Key Responsibilities
  • Assist with personal care and hygiene tasks, as required
  • Administer medication, as required
  • Support with preparing and cooking meals
  • Accompany customers to medical appointments
  • Support with social activities and independent skills
  • Assist with domestic duties, such as cleaning and laundry
  • Maintain accurate records and update care plans as required
Requirements
  • Full UK/International driving license
  • Access to a vehicle, with mileage paid at 0.45p per mile
  • Ability to work every other weekend
  • Willingness to achieve the Care Certificate
  • Good communication and interpersonal skills
  • Ability to work independently and as part of a team
