About the Role

Suffolk CRHTT is undergoing a period of transformation under new operational leadership and line management, making it an exciting time to be part of the team. We are seeking experienced mental health professionals to join our Crisis Response and Home Treatment Team.

Key Responsibilities
  • Provide care, treatment, and assessment to patients in crisis, both at base and in their own homes.
  • Work within a multidisciplinary team to manage risk and support service users in West Suffolk.
  • Develop and implement care plans, including discharge planning, risk assessment, and admission prevention.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Participate in transformation agenda to improve patient outcomes and service delivery.
Requirements
  • Nursing degree or equivalent qualification.
  • Ongoing professional development working towards degree level.
  • Experience in mental health practice, with a focus on crisis care.
  • Effective risk assessment and risk management skills.
  • Able to travel independently.
