About the Role

We are seeking a dedicated and detail-oriented Supporting Families Practitioner to join our team at HCRG Care Group. As a key member of our Family Wellbeing Service, you will play a crucial role in reviewing patient health records and ensuring their alignment with the National Supporting Families Outcome Framework.

Key Responsibilities
  • Conduct systematic reviews of patient health records to identify relevant data that aligns with the framework's criteria.
  • Ensure the accuracy and completeness of health information in relation to the framework's outcomes and indicators.
  • Complete reports summarising the alignment of health data with the framework's criteria.
  • Work collaboratively with system partners to integrate relevant data into Patient Health records to accurately support National Supporting Families Outcome Framework.
  • Adhere to the service referral protocols and escalate cases where the level of need becomes beyond scope.
  • Identify support needs and provide signposting for patients and carers families.
  • Participate in networking with all other relevant areas to enable a seamless pathway of care.
  • Stay informed about updates to the Supporting Families Outcome Framework and best practices in health data management.

Requirements
  • GCSE English and Maths, Grade C and above, or equivalent qualifications.
  • Experience within an administrative role or experience of working in a customer service environment.
  • Excellent communication and interpersonal skills, both verbally and written, to all levels both internally and externally.
  • Effectively prioritises and coordinates own work alongside work of others.
  • Standard keyboard skills.
  • Ability to use Microsoft Office packages.
  • Work flexibly as part of a proactive team.
  • Excellent time management skills.
  • Ability to use initiative without close supervision.
  • Good organisational and planning skills.
  • Understanding of good customer care.
  • Self-motivated.
  • Ability to remain calm in a busy environment.
  • Reliable.
