Job Title: Hospital Social Worker Bridgend

Job Summary:

We are seeking a qualified social worker to support the hospital social work function at the Princess of Wales Hospital. The successful candidate will work closely with the multi-disciplinary team to assess patients on hospital wards and facilitate discharges.

Key Responsibilities:

  • Assess patients on hospital wards and develop discharge plans
  • Work closely with the multi-disciplinary team to ensure seamless care transitions
  • Collaborate with patients, families, and healthcare professionals to achieve positive outcomes
  • Utilize the WICCis system to track patient information and facilitate discharges
  • Apply the principles of the Mental Capacity Act and identify triggers for CHC

Requirements:

  • Minimum of 6 months experience of working in the UK as a registered social worker
  • Relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
