- Develop and implement individualized medication plans to optimize patient care and outcomes.
- Collaborate with healthcare professionals to ensure seamless patient care and transitions.
- Medicines Optimisation and Safety
- Develop and implement strategies to improve medication safety and reduce medication errors.
- Conduct regular audits and reviews to ensure compliance with medication safety standards.
- Education and Training
- Develop and deliver education programs to healthcare professionals and patients on medication management and patient care.
- Provide expert advice and guidance on medication-related issues to healthcare professionals.
- Quality and Performance Improvement

- Qualifications
- Master's degree in Pharmacy or equivalent.
- Registration with the General Pharmaceutical Council as a pharmacist.
- Experience
- Minimum 2 years of experience in a leadership role in a pharmacy setting.
- Experience in medication management and patient care services.
- Skills and Knowledge
- Strong leadership and management skills.
