Job Summary:
We are seeking a highly skilled and experienced Practice Nurse to join our team at NHS Scotland. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and managing chronic diseases.
Key Responsibilities:
- 1. Clinical Expertise: Manage patients with chronic diseases, running specific clinic sessions and maintaining robust call and recall systems.
- 2. National Screening Work: Participate in national screening work such as cervical cytology.
- 3. Chronic Disease Management: Monitor and manage chronic diseases based on protocols, including relevant prescribing changes.
- 4. Diagnostic Testing: Select, carry out, and interpret results of specific tests.
- 5. Contraceptive Services: Provide directly and assist GPs with contraceptive services.
- 6. Chaperone Services: Act as chaperone when required.
- 7. Health Promotion: Undertake health promotion to help educate patients.
- 8. Supervision and Support: Supervise and support HCA posts.
- 9. Clinic Management: Undertake clinics to include interpretation of blood tests and dosing.
- 10. Multidisciplinary Team Meetings: Participate at MDT meetings.
Administrative Responsibilities:
- 11. Patient Records: Maintain accurate patient records and enter onto the computer using agreed Read Codes.
- 12. Documentation: Ensure accurate completion of all necessary documentation associated with patient healthcare.
- 13. Data Collection: Collect data and participate in occasional Practice audit and research projects.
- 14. Practice Development: Contribute to the preparation of any Practice development plans.
General Responsibilities:
- 15. Professional Registration: Maintain own annual registration with the NMC and adhere to Code of Professional Conduct.
- 16. Safe and Therapeutic Environment: Maintain a safe and therapeutic environment for all PHCT staff, patients, carers, and co-workers from other agencies.
- 17. Professional Approach: Promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
- 18. Continuous Professional Development: Maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
- 19. Staff and Practice Meetings: Attend and participate in staff and Practice meetings and in-house clinical training sessions.
- 20. Reasonable Duties: Perform any other reasonable duties that may be required from time-to-time.
