About the Role

We are seeking an experienced Occupational Therapist to join our dynamic community team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality care to patients, working closely with healthcare professionals to develop and implement effective rehabilitation plans.

Key Responsibilities

  • Conduct thorough assessments of patients' needs and develop personalized rehabilitation plans
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Provide education and support to patients and their families to promote independence and self-management
  • Manage a varied caseload, including post-operative rehabilitation, long-term condition management, and falls prevention
  • Develop and maintain strong relationships with community partners to ensure effective care coordination

Requirements

  • Registered Occupational Therapist with a strong background in rehabilitation and community care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Strong problem-solving and prioritization skills, with the ability to manage a high-volume caseload
  • Sound knowledge of healthcare policies and procedures, with a commitment to delivering high-quality care
