Position Overview

As a Clinical Pharmacy Associate, you will play a vital role in our network of GP Practices, collaborating closely with Practice-based Clinical Pharmacists to facilitate the safe and effective management of medication. Your contributions will be essential in enhancing patient care through optimized medication use and minimizing waste.

Key Responsibilities

  • Engage with patients to support their medication management, ensuring clarity and understanding through shared decision-making.
  • Conduct medication optimization tasks, including reviewing inhaler techniques and performing medicines reconciliation.
  • Assist in the management of medication reviews for new care home patients and coordinate medication transfers between care settings.
  • Provide expert advice on public health and social care needs, including lifestyle modifications and addressing local health disparities.
  • Participate in initiatives aimed at promoting responsible antibiotic use and reducing unnecessary prescriptions.
  • Contribute to the implementation of medicines optimization strategies and patient safety evaluations.

Essential Qualifications

  • Registered with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician.
  • Completion of a BTEC/NVQ level 3 or equivalent in pharmacy services.

Desirable Skills

  • Experience with EMIS and community pharmacy systems.
  • Familiarity with medicines reconciliation processes.
  • Understanding of quality frameworks and incentive schemes.
