Inpatient rehabilitation facilities offer patients a chance to engage in intensive recovery within a supportive community environment, backed by round-the-clock nursing assistance. These facilities play a crucial role for individuals who do not require hospital care but are not yet ready to return home.
We are seeking Physicians to join our rehabilitation teams in Central London. As a Physician in these settings, you will collaborate closely with nursing staff and therapists as part of a comprehensive healthcare team, overseeing the medical care of patients throughout their rehabilitation process.
Key Responsibilities
Our patient demographic primarily consists of elderly individuals who have transitioned from acute hospital care and are medically stable yet physiologically delicate. This role presents an exciting opportunity for Physicians interested in geriatric care, rehabilitation, general practice, and acute medicine, with a commitment to working within a multidisciplinary framework.
Your duties will include working during daytime hours on weekdays, with weekend coverage on a rotational basis. In addition to clinical responsibilities, you will support colleagues, engage in educational initiatives, drive service enhancements, and liaise with both hospital and community professionals.
This position allows you to lead a service at a time when healthcare systems are increasingly emphasizing integration, enabling you to contribute to team development and foster connections with community teams.

Job Duties Include:
- Conducting thorough medical assessments and reviews for all patients in the units, including new admissions and those experiencing stability or deterioration.
- Ensuring accurate prescriptions and appropriate management tailored to the patient and service context, while promptly reviewing results and correspondence.
- Coordinating medical care by collaborating with community and hospital colleagues to maintain continuity of care and escalate cases to acute settings when necessary.
- Documenting in accordance with professional standards and organizational policies, including clinical records and communications such as discharge summaries.
- Facilitating effective communication with admitting clinicians regarding your area of responsibility.
Qualifications:
Essential:
- Professional registration.
- Completion of training in General Practice, Geriatric Care, or Rehabilitation Medicine.
Desirable:
- Leadership qualifications.
- Training in mentorship or supervisory skills.
- Relevant post-registration qualifications (e.g., MRCP, DGM).
Experience:
Essential:
- Experience as a healthcare professional in a community setting, including GP practices.
Desirable:
- Experience with frail patients.
- Prior work in a community hospital, rehabilitation service, or frailty service.
- Experience in areas of social deprivation.
- Leadership in improvement projects or programs.
- Cross-organizational healthcare experience.
- Experience in developing clinical governance.
Skills & Knowledge:

- Strong understanding of older adults, acute medicine, frailty, and rehabilitation.
- Proficiency in Microsoft Word and GP practice systems.
- Aptitude for interpreting complex or incomplete information.
