About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our team in delivering high-quality care and support to individuals with life-limiting illnesses and their families.

Key Responsibilities
  • Provide one-to-one care and support to patients in their own homes, focusing on their physical, emotional, and social needs.
  • Assist patients with daily living activities, such as personal care, medication management, and symptom monitoring.
  • Offer emotional support and companionship to patients and their families, promoting a sense of comfort and well-being.
  • Work collaboratively with the multidisciplinary team to develop and implement personalized care plans.
  • Contribute to the maintenance of accurate and detailed records of patient care and progress.

Requirements
  • Relevant community experience and a strong understanding of end-of-life care principles.
  • A S/NVQ Level 2/3 qualification or equivalent certificate/diploma is desirable.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • A compassionate and empathetic approach to patient care, with a focus on delivering high-quality, person-centered support.
